A: No it does not include the coin. Only if you pre-order it. But you can get it when you buy strategy guide.
A: No you had to pre order and pick it up that first week of release, and since it was a pre order bonus outside of the (not included in) game bundle it came in limited quantity so only option is to buy it separately if you really want the coin.
