They are basically identical visuallly and with what they can do except for one feature, the series 5 has an always on display so that you can always see the time. It does affect battery life if you’re using the watch heavily all day, I recommend disabling the always on on days like that. Otherwise I get a full 18 hours of day use with it on and still have 40% at the end of my day.

I understand 4 did many of the same things as 5, including the first to monitor ECG. However, I got the feeling that 4 had some shortcomings (unknown to me) and was somewhat of a beta version of 5. That's just my feelings however.
