A: no extra power required. it is a standard length card that will fit in a pci slot. i have a cooler master haf 912 case and have no issues. the card is wider than some cards you might lose a slot due to the cooling on the card

A: It looks like a full size slotted tower. That should physically fit with no problem.
A: The specification is correct. The max supported resolution is 1920 x 1080. The HDMI version is 1.4a & so even if the resolution is set to 3840 x 2160, it will not support 60Hz. A GPU that supports HDMI 2.0 and up is required for 4K.
