A group of adventurous young friends on a trip to the Caribbean decide to explore a secluded island, where they unleash a highly contagious, flesh-eating virus after swimming in contaminated water, and exploring a research facility that houses a terrifying secret. Somewhere on an uncharted island deep in the Caribbean, the doctors at a secluded research lab have managed to quarantine Porter (Sean Astin), the first person to be infected by an aggressive flesh-eating virus. Meanwhile, not too far away, Marcus (Mitch Ryan) is preparing to get married when his brother Josh (Brando Eaton), Josh's girlfriend Penny (Jillian Murray), and their best friend Dobbs (Ryan Donowho) lure him onto a luxury yacht to celebrate his last night of freedom. Upon arriving at a picturesque island that isn't on the maps, Josh and Penny stay on the beach to do a little snorkeling while Marcus and Dobbs set up the campsite. Returning to the tent in a panic after spotting scores of dead fish in the water, Penny breaks out in a rash that Josh credits to some bad local ganja. Later, when Josh also shows signs of infection, Marcus and Dobbs venture out for help and break into the darkened research facility, which appears to have been the scene of a bloody struggle. Their worst fears are realized when they discover that they have stumbled upon the site of a deadly outbreak, and that the egomaniacal doctor (Currie Graham) in charge is determined to take credit for curing the disease -- even if it means that no one gets off the island alive.~Jason Buchanan

3rd times a charm?
After the horrible Cabin Fever 2 I figured this would have to be better and it is. While not a great movie, it has more of the feel of the first film than the second. It also has one of the best gore/fight scenes ever seen. I wouldn't recommend it to anyone who isn't a fan of the first film or the genre though. I've seen it dissed plenty online but found it to be a fun worth-while sequel in my book.
the movie was better than expected since it was a direct to video release if it was in theaters it was very limited. I really liked the first movie in series and this movie had good production values and decent acting. i also liked the premise of the secret hidden facility where all the action takes place. the fx were truly awesome.
While I wasn't expecting much coming into this movie, I bought it simply because I am a huge fan of Cabin Fever (the original). While the graphics were alright the storyline lacked and got confusing at parts. With that being said, I got basically what I was expecting. I would watch it again.

Graffic, With a Great Twist!
This was actually a surprisingly good film. Most times when these "sequel/prequels" come out years later they are terrible. Sean Astin plays a good role. Great if you like some good old fashioned gore! The best part is the twist. You'll have to watch it to know what I mean...
Cabin Fever Patient Zero is a nice addition to the running series. This film has some decent gore scenes and looks really good but I felt the acting was just average and the story was left wanting. Does have a good twist at the ending. Overall it is not better than the original but a little better than the second installment.
