“The AppleCare+ limitations say, "If your Mac has catastrophic damage from an accident ... you will need to buy a replacement." Does this mean no coverage in this case?”

The replacement purchase is about $200 ( I believe) in case your computers is unable to be repaired. Which is a lot less compared with the price you pay for a regular Mac. So the Apple Care still covers you. Basically, you don't pay a full price only the deductible. I hope I clarify your question.
