For your requirements, I suggest trying the Bose Ultra with different ear bands; small supportive wires to wrap the exterior ear. Or maybe different brands. Google Pixel Buds are smaller in size and might fit your ear cup better.

Unfortunately not. Only the inner ear piece is customizeable.

Hello Sally, different eartip sizes are included with the QuietComfort earbuds (small, medium and large). I would recommend trying the smaller size (size 1).
