“Does this have a QAM tuner? Has anyone had success getting basic cable channels with a direct coax connection without a cable box? Clear QAM tuner picks up the 20-40 channels not scrambled by the cable providers. (I know it has an ATSC OTA antenna tuner.)”

Yes, this TV has a built-in QAM tuner. We recommend connecting a Cable Box to one of the HDMI inputs on the TV.
