“My middle shelf keeps slipping out of the bracket and falling. I do not have too much weight on it (only a PS4) and yet its fallen twice. Does anyone know of a fix for that or should I return the stand?”

The round rubber adhesive dots that stick to the frame around the perimeter of glass should keep it from sliding.
