• Yes it does, and can be connected to the tripod in 2 planes as it has 2 locations to screw into.

• It does come with that adapter. It clamps onto the phone with a spring, which seems strong enough, but there is no lock or latching mechanism. It is small enough to hold my original iPhone SE. It is too small to hold my wife's iPhone XS in its case; it would probably barely hold it if we took the case off, but we haven't tried that. If you have a bigger phone and/or a big case this adapter may be too small.
