What cpu cooler and power supply does this have null

The cooler is a 120mm AIO Radiator and is equipped with a 650W 80 PLUS Gold power supply unit.

I had two break within 6 weeks of purchase for the cpu cooler. Which when broken cause the cpu to overheat and restart the computer.
