No. Contact the carrier directly to have fee waived

I called verizon myself and asked them to waive the activation fee and they did.

Some carriers will run new upgrade with no activation fees. Make sure to pay very close attention to your bill...have gotten charged in the past

Only when they are running deals and those deals are always stated on the page.
