“Does anyone know if Best Buy and/or Verizon include Samsung's Premium Care in their contract? I saw that if you buy from Samsung direct you get this (wanted or not) for an extra $11.99 month without realizing you are signing up for it.”

No one is forcing you to take the care plan, you can the phone without any care plans. Samsung gives you the option for their care plan or non care plan. As there are 3 selections from their website. Now Verizon has its own care plan and Best Buy has its own as well. Just don't select them if you don't want it.
