Technically no, as this is portable if you wanna move it from room to room. But if you want to avoid moving it around, or would want simultaneous use on multiple tvs, then yes a Roku per tv.

Yes, this connects to a spare HDMI input on your TV or Receiver. It does not have any recording capability.

Nan, Roku is designed to work with one TV at a time. You can move them between TVs, but I would recommend doing that a lot. I move a device outside once and awhile for movie night, but I wouldn't want to do that everyday. 
Recording programs isn't built into Roku however a lot of our channel partners offer Cloud Based DVR services like Hulu or YouTubeTV.
