When I pre-ordered the game back in January, it wasn't included in my order either. I got an update email 4-5 days later saying they noticed it hadn't been included and that they had added it to my order. I'm not sure if it's a limited amount or what, but this was just my experience. I actually didn't even know there was a pre-order bonus before I got the update.

It's still listed on my purchase, but I have no idea when it may be delivered-- Reddit is saying that it might be shipped individually from the game.
