Im answering my own question in case it's helpful to anyone else. Make sure the Yamaha receiver HDMI input is set to "MODE 1." Make sure your TV E-ARC HDMI input / output is set to "enhanced" mode (Sony TVs) or whatever option enables 4k / 60hz inputs.

I don't know if it will help, but I had trouble with Yamaha connection to my new Vizio tv (4K-8K capable) and I switched to a Rocketfish "Ultra High-speed" HDMI cord (from the tv's HDMI ARC connector to the Yamaha receiver), and it took care of the problem.
