Rather than submit a help desk ticket and hope that someone acted on it quickly, I chose to order a new one from BestBuy and pick it up. It has done what I expect it to do. Well done!

NOTE: If you are attempting to add a 3rd monitor, and you already have 2 HDMI devices (direct or with other passive adapters) and want to use a 3rd hdmi monitor, don't buy this. Get an active adapter.
