You do not have to activate the cellular on your watch. I recently bought the Series 4 stainless steel Apple Watch. I bought this particular watch primarily for the stronger watch face, not really for the cellular. I have worn my watch for about a month and not activated it with a carrier. As long as you don't activate it with a carrier, you don't have to pay anything extra. Activating it with a carrier means you can wear it while not having your cell phone and still take calls, answer texts, listen to music, etc. Not activating it means your cell phone must be close to you to do these things. Different carriers charge different amounts (I believe). I think my carrier would charge me $10/month to activate the cellular.

There is a $10 line access a month fee to have it attached to your same phone number or $20 a month to have a standalone phone number on the watch. If you purchase the watch full price at once through Best Buy, you will only be billed the line access fee from Verizon to have it added to your account. If you decide to finance it instead through Verizon, then you will be billed the monthly installment PLUS the line access fee.
