No it does not. You have to assemble it yourself. It takes about 30-40 minutes. It comes in a rectangular box.

It does not. Assembly took me about 45 minutes. Hardest part was a rubber membrane that had to be stuffed around the monitor. Besides that it was easier than other arcade 1ups.

No it does not. But it was very easy to put together. 30min to 1 hr tops.
