“It looks like if I purchased the warranty on an item say a samsung galaxy watch then everything is covered 100% labor and parts. However through my total tech it looks like there is a service fee if I use it. That was never disclosed to me.”

The employees at store won't tell you, but it is in the online information kind of buried. If after warranty they charge, they will also tell you to go to the manufacturer if under warranty, makes no sense.
