• No it does not. But those cords are a dime a dozen. Any micro USB cord, be it from an old cell charger or anything will work.

• No. Like any other controller it doesn't include an USB charger. I would recommend the skin pack that comes with the USB charger and controller skin for $10 or so.

• No, a usb cable is not required. However, micro USB cable are very common nowadays and extremely easy to find.
