It gets your heart rate while wearing it on your wrist. I don’t know why a pacemaker would be any issue.

You do have to wear the watch, but you do not need an extra strap for the heart rate monitor. The are some green flashing lights on the bottom of the watch (the side towards your skin) that I believe are used to monitor the heart rate. I am not a doctor so I can't really answer the pacemaker question (you should talk to your doctor) but I think it should be no riskier than carrying your cell phone.
