No. It works with several free options including tune in radio, pandora, and iheart radio. You must set these up on your phone

Yes and no. You can search for a current list of apps that integrate their functionality with Google Home, and many of them have free levels. There are also free radio apps. I don't pay for music at all, and am happy with what I get. Remember that you can cast from the Chrome browser to your Home device, too.

Essentially, you can't necessarily use voice commands to play every app, but you can find solutions to get almost anything playing.

No you do not. You can add services that you subscribe to, such as Spotify or Pandora. But I do not subscribe to a specific music service, and I listen to music on the Google Home all day long!

I daily use a free Spotify and Pandora account to stream music. Just the occasional ad as per usual.

Or you can send music you may have on a device.
