When and how is a phone number assigned? null

My phone number was assigned when I activated the phone through Lively.

Great question! At the time of activation you are assigned a Lively phone number, however if you wish to Lively allows you the option to transfer your cell or home phone number, as long as your number is still available. Just make sure you do not cancel your previous service until the transfer is complete.
