I do not believe you need to download anything to play but it has plenty of updates. Updates have included new modes and more characters.

No. Should install without an additional download. Updates and dlc require additional download but the base game does not.

There is optional DLC content that you have to pay for. This paid content gets you extra characters and stories over the next few months or so. There is some free extra content, but it comes up the first time you go back into the game after it is available. I don't know if I would call that a download, as it happens instantly once you go into the game.

There's a download, but I believe it's just a patch, not a large file.
