This device does pass through a surround signal, but stereo Bluetooth headphones will not support surround sound. Essentially, this means you will have to change the optical audio output format in your TV's sound menu to stereo/PCM when using Bluetooth headphones, and then change it back to Dolby/surround when using your surround system. You will not need to adjust any cables, just the menu setting.
