yes you can extend your subscription by going to your PlayStation Plus membership and clicking down below I'll have pictures to show where you can extend so you can extend it through PlayStation Plus as you can see so you should be able to stack with a digital cards as well

Yes, activating a new code should add the codes length (cards are offered in 3, 6, and 12 months) to your current membership. You don't have to wait for your current time to expire before redeeming the new one.
