No they don’t it’s just that the durability of that weapon has run out. Some weapons have better durability then others, so try finding better weapons or just carry multiple weapons.

They can break in the middle of combat which is frustrating at times but they allow you to switch weapons in the middle of battle very easily while also stopping all action while you do. Great game mechanics.

Yes, so stock up on both melee weapons and bows. They'll both break during combat but you will get a message that lets you know they are badly damaged, which means they'll break within the next 2 or 3 shots.
