Yes it does and I recommend you use it because it enables the Amplified Stereo Mode and Mic Monitoring. When you just use the 3.5 mm Jack it doesnt enable that.

The USB adapter is included in the box. I've never used it, but it has the USB plug in one end and the audio jack in the other.

It connects to your PS4 via USB, but you can also unplug the headset from said USB stick to use as regular 3.5mm headphones
