Couple of solutions here. If you are on an Xbox One or Xbox 360 it could be either the female insert on the bottom of the controller or a bad headset (headset adapter as well for the Xbox one) hard to tell from your question. The headset has a on/off switch where the volume control is. 2.- Do a controller firmware update: remove the batteries on your X1 controller and connect it to the console using a micro-usb cable (it should be included on the Xbox One's box). On System Config, you should be able to update the controller's firmware.
