This card was added to my online planned purchase for 4 appliances (dishwasher, over range microwave, double oven range, and refrigerator). I didnt buy online and purchased from the lical store (purchase is verifiable) and they did not include the promotional card.

Your customer service team should be able to resend it to your customers or at least escalate it to the appropriate team to handle.
