There is a fee if you choose to use the premium features. You do not need the premium features if your cardiologist has a pro account and provides you with a code. I am able to submit my Ekg results to my cardiologist with the share feature in the app. 
You are able to try out the premium features for thirty days, that should give you enough time to decide if you need them or not.

No, But there is a premium plan that will track medicine, 
replace device if lost or stolen for $20, 
a monthly report and a couple of other things. 
You do not have to buy.
