The Apple Watch Series 4 has the EKG/ECG hardware already installed. Later this year, Apple will release a software update enabling EKG/ECG capabilities.
Coming in a later WatchOS update (probably 4.1). This feature is not active at launch date.

No ones watch does yet. That comes out later this year.
It has an electrocardiograph sensor built into the Digital Crown, however it isn’t enabled yet because they are finishing up the app for it. Should be realeased soon tho.
