• Yes it can. It comes with instructions for either method. It even a sort of stencil to help with alignment.

• If you can make a good hole for the screws in your concrete, then, sure. Putting up a couple of 1 foot 2x4s first might be a reasonable idea. That would hold this mount, and the TV, out a bit from the wall, making sure that the TV's input and power cables have clearance.
