You have to have an existing internet connection with an ISP to access the internet on your network. That connection is then used by the router to provide a connection to any attached devices. This router itself is a one time purchase that you connect to your network. There are no other fees associated with it. You can use the router by itself to allow devices in your house to communicate to each other, but to connect to the internet, you need to contact an Internet Service Provider. This is commonly provided by a modem that the ISP connects to and gives you an ethernet connection that you can then connect to the router.
