“The instructions/dimensions on Page 7 say "maximum depth 12." That can't be right? How deep (minimum) should the cabinet above be?”

If you’re using the recirculating fan setup then it can’t be any deeper than 12 inches. If you’re using the ducted option then you can do as deep as 16.5-17 inches
