“The description of this Samsung StormWash model #DW80K5050UW states it comes with cutlery rack, as well as upper and lower racks. I would assume the upper and lower racks are included and not accessories. Did anyone receive the cutlery rack?”

2 main racks (upper and lower); and 2 cutlery racks that fit into the lower rack. But I don’t recommend this machine! Bought it 11 months ago and have regretted my choice ever since.
