“The device only works when using the headphone jack. I tried using the audio output jacks on two differnet tvs and it won't work. Green light keeps flashing indicating an issue. I need to connect to the audio so that others can hear the tv.”

I have my earphones connected to AUDIO OUT jacks and they work fine. TV and earphone volumes work independently. Should work for you if properly connected.
