“I need to take my home telephone wiring out of the equation. Can I plug the Vtech base station directly into my Optimum cable modem (Rj 45), get dial tone, and then have the base station communicate to the other cordless handsets?”

Hi - The VTech IS8151-4 only works with an RJ11. It is not compatible with an RJ45 telephone jack. Thank you, -VTech Support
