You have to call their plan service number or enter a claim on their plan claim website. I found it to be a big rip-off. 
I paid over $160 for this so called protection plan. There's a small crack in the bottom corner of the screen as well a a few other technical issues. I took the phone to Best Buy to have it fixed or replaced. I was told I had to pay a few of over $200 with tax. Over $360 for a protection plan repair or refurbished phone. You can get a new one on Amazon for $349! To have it fixed for the original $160 was doable. To charge an additional $200 is a rip off!

Don't bother unless you are in one of only 12 states that offer the program. Based on other comments, they aren't very good.

Just walk into any Best Buy and speak to a Geek Squad agent. They will gladly take you through the process of getting your phone issue fixed.
