Yes, the coaxial cable should be hooked up to the cable INPUT or the HDM1 INPUT located on the back of your tv normally.

Yes to your tv only other connection will be the power plug. Switch tv settings to antenna from cable and run the scan for channels

Yes, the coaxial cable from the antenna is to be connected directly to tv. Plug card into electrical outlet, scan for channels in "live tv" position on smart tv.
