Plugs right into the back of your XBox with a USB port. No need to hook up to the controller. All controls are built on the back of the headset. I HIGHLY RECOMMEND these headphones!

No it does not, there is a base unit that plugs into the xbox itself and the headphones are wireless and without a cord so it does not have to be wired to anything including the controller port

Does not connect to the controller. It connects to one of the USB ports on the xbox directly.
