That offer is exclusively through Samsung. You will need to order it through them directly. It would be for the unlocked version and not a carrier specific one.

Samsung will give you up to $425 when you use your current phone as a trade in depending on the phone you have, but you have to order through Samsung .com.

I just spoke with Samsung's customer service and if you are on a payment plan with Verizon, that must be payed off in full before you can send Samsung your phone to get the credit. You cannot just pay off half to upgrade like you can if you are turning it in to where you got your replacement. You don't gain anything if you owe more then the $425 dollar credit they are offering.
