it just works on its own. I had a jar to my body , but did not fall. It said to me. " it looks like you have fallen" then it offers to call for help. It gives you the option if saying you are ok and you can cancel the emergency call . if you don't answer it will call for help

Similar to a smartphone, you can scroll up and down on the screen with your finger, you can tab to activate buttons, and you may need to swipe (horizontally) to trigger certain functions (such as a shutdown when you are rebooting it). Once you have all of that down, the fall alert is similar to any alert in that you will feel the vibration of the watch if it detects a fall. It will tell you that it believes you have taken a fall and will offer you the option to say "I'm ok" or "Call Emergency Services". If you do not respond to either (such as becoming unconscious), after 30 seconds, it will start to sound an alarm to try to get your attention or other people around you. This will get louder and louder and if no one responds to it, it will automatically call Emergency Services and the 911 operator will know that the call is initiated from an Apple Watch.
Sorry, there was a problem. Please try again later.I fell in the yard and immediately got a notification- “it looks like you may have fallen’. It gives you the option to call 911 or cancel. If no response is a specified time 911 will automatically be called. My fall did not require any assistance so I canceled it. For me it worked great.
