This could be a simple as the USB being damaged. First check to see if it will work on any other PC. You should also try other USB ports on the same PC. If the issue follows the USB, then it was not removed correctly or it shorted out and you will have to replace it. Another thing to try would be a different USB on the computer in question (the original one used). If the issue follows the PC, then is the USB ports and you should have a IT guy/gal take a look at it for you to see if it is physical damage to the hardware, or software (including virus).
