You do need to log onto Google to complete setup. There is no charge for this and the URL is listed in the setup documentation that comes in the package.

There is an app for the phone that can be used to set up the chromecast initially. This can also be done via computer. After it's set up, then the functionality is embedded in the app you want to cast. For example, once set up, in order to cast Netflix, you only need the Netflix app (on your phone or tablet). This can also be done from Chrome on your computer, without any phone interaction at all. All apps are available in the Play store.

No, you plug it in and go to the website online and set it up from there. While there are apps that can automatically "connect" to the Chromecast, you can simply use it through Google chrome on your computer.

You'll need the app for your phone/tablet, and you can get it from the play store. You'll also need the chromecast extension for your chrome browser, if you want to cast from your computer. While you're at it, you should grab the videostream app and extension, so that you can stream video files from your computer to your chromecast device. It works really well.
