Yes you will need 1 box per tv.

It depends. If you want to be able to watch 4 different channels simultaneously on the 4 TVs, then yes, you need 4 boxes. If you only watch 1 tv at a time, then you could split the signal coming out of the Orby receiver to the 4 TVs keeping in mind they will all display whatever channel the receiver is tuned to.
