• Yes, unless you already have a preexisting POE switch connected to your router. The adapter turns your standard ethernet port into a powered ethernet port, i.e. POE.

• Yes, a network patch cable needs to connect the poe adapter to your router. You'll also need to plug in to an outlet.

• I don't own this camera but in my experience Yes. You will need to run an ethernet cable from your router to the IN port on the PoE adapter then run an ethernet cable from the OUT port on the adapter to your camera. Unless you have PoE ports on your router. You could then just run a cable from your router to your camera.
