“I have an old Sony home theater receiver. My TV is connected to it via the optical port, so sound for all of my tv devices are controlled that way. If I connect this via the audio out port, will I have to switch to that on the receiver to hear sound?”

Yes - I have a similar set up on my Sony system. My TV/DVD run through Input 1 and digital audio input/output, and since there is only one digital audio input spot on the Sony, I set up the Bluetooth receiver using the RCA type output and ran it through the Tape input on the Sony receiver. Although audio output is analog for the Bluetooth with how I set it up - it still sounds really good - and I don't mind having to change the Sony unit from Input 1 to Tape as needed. to hear the Bluetooth.
