“I just bought a 55" TV from Best Buy and added 2 yr service. Is $59 total price or quarterly price? Do I need to make an apt for you to install and set up?”

The 2 year service is for when something breaks etc.. you have to call best buy n tell them you need it set up for installation and they will set you up a date!
