This uses Wi-Fi for streaming movies tv and such. It doesn't connect directly to a TV cable. If you have internet and Wi-Fi it will work.

Works well with Spectrum. There is a Spectrum app, but it is not on Firestick.

The Fire will connect via wifi with your Spectrum cable system. If by boxes, you mean the router, then you do need it. Unless you purchase your own, and if only for internet. If the box also runs your TV channels, then probably not.

You can use spectrum to login to your favorite channels on firestick such as hbo, showtime, cinemax, Fox etc. Go to the app and follow instructions to see if your service provider is listed.

No you can't use the Amazon fire stick as a cable box. The fire stick is for streaming movies from Netflix or Amazon prime , Hulu and other apps like that. 
You will need a internet service and Wi-Fi to use the fire stick.
